All affiliation request to IMAGE, CBWTF for Biomedical waste management is accepted only through Online Application.

Click "APPLY FOR AFFILIATION" http://imaimage.biz/Affiliation-register link to apply for new affiliation of your institution to IMAGE

Upload the scanned copies of following to the Online Application Form :

Terms & Conditions for IMAGE Service (download from the website, duly sign & affix the office seal on all pages)
Registration Certificate of Segregation in-charge
Letter of Undertaking signed by Segregation In-charge (the form of Letter of Undertaking can be downloaded from the website in Downloads tab)
Copy of the State Pollution Control Board consent form (if already registered with PCB)
Copy of Licence issued by the LSGD (if already registered with LSGD)
Other relevant documents, if any.

Once you click the SUBMIT button, the soft copy of the filled-up Application Form along with Terms and Conditions of IMAGE service, will be sent immediately to your given E - Mail address

You can track status of your application by clicking, "Request Status" of IMAGE Website
